The Statement of Intention to Visit is the primary planning tool for SAPAA Volunteers. It also serves as Hazard Assessment in compliance with SAPAA’s Safety program.

The Statement of Intent to Visit can be completed below or accessed directly via this link: https://adminsapaa.survey.fm/2025-intent-to-visit. The entire form must be completed and submitted to be approved. The last page is a waiver and an assumption of risk on behalf of the volunteer.
